By participating in the Clear Aligner Open Day event on Friday, 24th November 2023, organized by My Dentist at Cannon Hill in partnership with Smile Academy, you agree to comply with the following terms and conditions:
1. Eligibility:
- This event is open to individuals aged 16 and above.
- Persons under 18 must be accompanied by a guardian.
- Appointments for free consultations, 3D scans, and before-and-after simulations are subject to availability.
2. Free Consultation:
- Complimentary consultations are offered on a first-come, first-served basis. It is by appointment only.
- The consultation does not guarantee eligibility for clear aligner treatment.
3. Free 3D Scan of Teeth:
- The free 3D scan is offered on the condition that it will be used solely for the purpose of creating a personalized treatment plan.
- Participants must follow instructions provided by dental professionals during the scanning process.
4. Free Before and After Simulation:
- The before-and-after simulation is a visual representation and may not guarantee the exact outcome of the treatment.
- Participants understand that individual results may vary.
5. Exclusive Savings:
- To enjoy exclusive savings of up to $1500, participants must sign up for clear aligner treatment within 7 days after the event day by paying a non-refundable deposit of $500.
- The savings of up to $1500 are based on the comprehensive package, including free consultation, 3D scan of teeth, before-and-after simulation, upper and lower fixed retainers, 1 set of upper and lower retainers and teeth whitening during your clear aligner treatment. This saving may vary from person to person depending on the type of retainers required.
- This offer is subject to individual eligibility.
6. Event Details:
- The event will take place on Friday, 24th November 2023, at My Dentist at Cannon Hill.
- Appointments are by reservation only, and spots are limited.
- Participants are encouraged to arrive on time for their scheduled appointments.
7. Booking Appointments:
- Appointments can be booked by calling 07 3902 1023 or using the online booking system.
- Participants are responsible for providing accurate and up-to-date contact information during the booking process.
8. Changes and Cancellations:
- Any changes or cancellations to appointments must be communicated at least 24 hours in advance.
- Failure to provide timely notice may result in forfeiture of the appointment.
9. Data and Privacy:
- Participants consent to the collection and use of their personal information for the purposes of the event.
- All personal information will be handled in accordance with applicable privacy laws.
10. General:
- Smile Academy Clear Aligners and My Dentist at Cannon Hill reserve the right to modify or terminate the event at their discretion.
- Participants engage in the event at their own risk and acknowledge that Smile Academy Clear Aligners and My Dentist at Cannon Hill are not liable for any damages or injuries incurred during the event.
11. Flexible Financing:
- Treatments are available from as low as $49 per week, based on financing an amount of $5000 on a 2-year plan using a third-party finance company – Zip Money.
- Financing terms and conditions are subject to approval by Zip Money.
